How do I explain to my granddaughter I cannot hold her for 6 months?

I've just learned I have a hernia in my diaphragm. Im scheduled for surgery in March, but I've been told no heavy lifting for the foreseeable future. How do I explain to a 21 month old little girl that I cannot pick her up?
